1. Removed template applications, and replaced them with new programming

1. Fixed a bug which could cause Miva to enter an infinite loop with some
corrupted index files.

2. Fixed a bug which caused Miva to enter an infinite loop when processing
HTTP file upload requests originating from a Macintosh using IE 4.5.

3. Fixed a bug in the handling of NULL fields in ODBC databases.

4. MvPACK and MvREINDEX now move their temporary files over the original files
after the entire process has completed, rather than as each file is completed.

5. Added fsymlink and ssymlink builtin functions.  (Under Windows, these functions
always return -1).

6. Added USER and PASSWORD attributes to the MvOPEN tag for use with ODBC datasources.

7. Miva Mia will now stop processing a script if the browser's "Stop" button is pressed.
This is treated the same as if the application had timed out (as it is under UNIX).
